A 80-year-old Thai man presents with acute urinary tract infection and acute decompensated heart failure. 

• Underlying diseases: HT, DM type 2, CHB s/p pacemaker, TVD with denied CABG 
• During hospitalization, he develops acute pain and swelling of Lt wrist and fingers. 
• He experienced multiple episodes of intermittent arthritis of bilateral hands, wrists, knees and ankles in the past 4 years.

1.Plain films findings 

• Multiple well-defined periarticular erosions with sclerotic border with overhanging edge at Lt 5th PIP 
• Joint space narrowing at Lt 5th PIP
• Soft tissue density around Lt 5th PIP (suspect tophi)
• Chondrocalcinosis at Lt triangular fibrocartilage complex (TFCC) 
• Soft tissue swelling of Lt hand and wrist 

2.Provisional diagnosis:  Acute gouty arthritis, with chronic tophaceous gout 

• The imaging finding of the Lt 5th PIP joint is consistent with gouty arthritis.
• However, chondrocalcinosis observed at the Lt wrist is a feature more commonly associated with another crystal deposition disease, calcium pyrophosphate deposition (CPPD) arthropathy.
• If feasible, arthrocentesis of affected joints may be necessary to establish a definitive diagnosis and differentiate between gout, CPPD arthropathy, or other causes, including septic arthritis.
