Fundoscopic examination of the Rt eye 

Question: What is the provisional diagnosis from the fundoscopic examination?

Case:

A 71-year-old woman with history of severe mitral regurgitation, severe mitral stenosis, status post AVR and MVR since 2016, and rheumatoid arthritis, presented with acute fever and left ankle arthritis for 3 days.

• PE:  BT 38.3, hypopyon and injected conjunctiva of Lt eye, swelling, warm, and tender during passive and active movement at all direction of Lt ankle.

Endophthalmitis of the Lt eye

• Lt ankle arthrocentesis was done. 

Gram stain of synovial fluid from the Lt ankle:

numerous gram positive cocci in chain

• Blood and synovial fluid culture from the Lt ankle revealed Streptococcus dysgalactiae.
• Fundoscopic examination was performed and the result was as shown on top.

Question: What is the provisional diagnosis from the fundoscopic examination?
