A 31-year-old woman presented with ascites for 10 months and dyspnea 1 week PTA. Abdominal paracentesis was performed. 

Ascites profile: 

• Turbid, brown 
• RBC 1,241,000 cells/mm3 
• WBC 2,790 cells/mm3, N 45%, L 2%, macrophage 45% 
• Albumin 2.97, protein 6.58 (serum albumin 3.4 g/dL) 

Question: What is the provisional diagnosis?

ANSWERs

• Diagnostic laparoscopy: multiple peritoneal plaque and necrotic nodules at pelvis area, seen peritoneal thickening with attachment to the liver surface, fallopian tube and both ovaries can not identified, not seen adnexal mass, serosanguinous ascites 1100 mL

• Pathological report:

1) Peritoneum, labeled as "Peritoneum I", biopsy:

- Endometriosis with chronic inflammation and old hemorrhage. 

- Negative for malignancy.

2) Peritoneum, labeled as "Peritoneum II", biopsy: 

- Organized clotted blood with hemorrhage

- Negative for malignancy

3) Peritoneum, labeled as "Peritoneum III", biopsy:

- Endometriosis with chronic inflammation and old hemorrhage. 

-Negative for malignancy.

• IHC studies show strongly postive for CD7, PAX-8, ER, Vimentin, weakly positive for CD20
• IMP: Peritoneal endometriosis 

Diagnosis: Peritoneal endometriosis with suspected Catamenial pneumothorax
