ANSWERs

1.บรรยายผื่นที่พบ 

• Small pink papules at central of face, symmetrically distributed “ Butterfly-shape pattern” across nose and cheeks เข้าได้กับ Facial angiofibroma (Adenoma sebaceum) เป็น common skin presentation of Tuberous sclerosis complex
• อาการแสดงทางผิวหนังอื่นๆได้แก่ Shagreen patch (collagenous fibroadenoma), Ash leaf spot (hypomelanocytic macule)

2.การส่งตรวจเพิ่มเติม

• Brain imaging: MRI หรือ CT brain เพื่อหา Cortical tuberous, subependymal nodule, subependymal giant cell astroma (SEGA)  ซึ่งพิจารณาส่งเป็นอันดับแรกเนื่องจากผู้ป่วยรายนี้มาด้วยอาการชัก
• อื่นๆ เช่น Echocardiogram อาจจะพบ Rhabdomyomas
• CXR/CT Chest อาจพบ cystic lung lesion หรือลักษณะที่เข้าได้กับ lymphangioleiomyomatosis ซึ่งทำให้สูญเสียการทำงานของปอด และเสี่ยงต่อการเกิด pneumothorax
• Renal imaging เช่น U/S หรือ MRI เพื่อดู renal angiomyolipoma (AML) เนื่องจากเสียงต่อการทำงานของไตแย่ลง และมีโอการแตกเป็น retroperitoneal hemorrhage ได้
• การส่งตรวจ genetic testing: Mutation of TSC1, TSC2 gene

3.การวินิจฉัย:  Tuberous sclerosis complex

• Tuberous sclerosis complex (TSC) is a rare genetic disorder inherited in an autosomal dominant pattern.
• It is characterized by the formation of benign tumors in various organs, including the brain, skin, kidneys, heart, and lungs.
• TSC is caused by mutations in either the TSC1 or TSC2 gene.
• Neurological presentations include seizure, autism, developmental delay, intellectual disability and SEGA.
