A 25-year-old woman presented with chronic headache for 5 months and blurred vision of the right eye for 1 month. VA RE 6/60 with positive RAPD, LE 6/9. Fundoscopy examination of the right eye was as shown.

ANSWERs

1.Diagnosis:  Papilledema 

• Swelling of optic disc due to increased intracranial pressure 
• Fundoscopy findings of papilledema: 
  • Elevated optic nerve 
  • Margin blurry 
  • Vessels obscured 
  • Venous dilatation and tortuosity
  • Peripapillary hemorrhages and exudates 
  • Leakage on fluorescein angiogram

2.Possible causes of papilledema:  

• Expanding mass lesion
• Cerebral edema due to stoke
• Intracranial hemorrhage
• Compression of ventricular system in posterior fossa
• Venous sinus thrombosis
• Infectious etiology e.g.cryptococcal meningitis 
• Pseudotumor cerebri

In this patient, CT brain with contrast was performed and the result showed a 5-cm. hyperdensity mass at Lt sphenoid with obstructive hydrocephalus.

• The patient underwent craniotomy with tumor removal.
• Pathological result revealed invasive meningioma.
