A 71-year-old Thai woman, with the underlying disease of dyslipidaemia, presented with acute dyspnea with chest discomfort for 1 day. Vital signs were stable, but the patient needed oxygen cannular 3 LPM to maintain oxygen saturation of 99%.

• Chest x-ray was normal.
• EKG was done (as above).
• Trop-T was positive.
• Echocardiography revealed good left ventricular function without region wall motion abnormalities with right ventricular dysfunction.
