A 58-year-old woman with SLE presented with chronic Rt hip pain during walking for 1 year.

• She was afebrile.
• The patient was diagnosed as SLE from the age of 33 years old.
• She had been on several medications, including corticosteroids since the time of diagnosis.
• Now, her SLE status had been in low disease activity state for 6 months.
• Her current medications included prednisolone 5 mg/d, azathioprine 75 mg/d, hydroxychloroquine 200 mg/d, calcium 1 g/day, and calciferol 40,000 IU/wk.

Films of both hips are as shown above.

1.Abnormal findings of the films of hips

• Crescent-shaped, curvillinear, lucent line at subchondral bone of femur, more prominently seen in the frog leg film
• Sclerotic change of femoral head 
• Flattening of articular surface of femoral head, bone collapse 
• No joint space narrowing 

2.Provisional diagnosis  Osteonecrosis or Avascular necrosis (AVN) of the Rt hip
