• Alveolar hemorrhage in immunocompromised patient (HIV)
• Film CXR: rapidly progressive bilateral alveolar infiltration compatible with alveolar hemorrhage

Differential diagnosis

• Infectious causes
  • Bacterial pneumonia > S.aureus
  • Viral pneumonia > Influenza , covid
  • Systemic infection > leptospirosis, dengue
  • Parasite > strongyloides 
  • Fungus > IPA
• Non-infectious causes 
  • Small-vessel vasculitis
  • ANCA-associated vasculitis
  • SLE with pulmonary hemorrhage
  • Goodpasture syndrome
  • Catastrophic APS
• Bleeding due to coagulopathy, warfarin overdose from DVT treatment

Investigations

• Sputum AFB:  Filariform larva strongyloides with notched tail

Final diagnosis:  Severe strongyloidiasis (hyperinfection syndrome)

Comment:

• Septicemia by enteric pathogen can be associated with hyperinfection syndrome.
• Adrenocortical carcinoma can produce excessive endogenous steroid.
• Excessive endogenous steroid increases risk of strongyloides infection.

Treatment

• For hyperinfection with sepsis or meningitis in severely immunocompromised hosts:  Ivermectin 200 ug/kg per day orally until stool and/or sputum exams are negative for 2 weeks.
