A 28-year-old Thai woman with no underlying diseases developed acute fever with cough.

While waiting to be examined by a physician at URI clinic, she developed sudden onset of right hemiperesis with motor power gr IV/V at the right upper and lower extremities.

She had experienced the same symptom about 4-5 years ago after drinking alcohol and it spontaneously resolved.

- ขั้นต้นประเมินคุณภาพ CT

• เคสนี้นักเทคนิคได้จัดวางภาพที่ค่อนข้างสมดุลซ้ายขวาอย่างดี โดยดูที่ eye globes ซึ่งไม่ได้แสดงไว้
• จากภาพจะพบว่าสมองซีกซ้าย เมื่อเทียบกับสมองซีกขวามีความแตกต่างอย่างเห็นได้ชัด white matter ของสมองซีกซ้ายหายไป แล้วถูกเติมเต็มด้วย inhomogeneous with multiple spot calcification content ซึ่งพื้นที่ดังกล่าวกินพื้นที่กว้างสู่ vertex จนไม่สามารถ identify suci และ gyri ได้
• ทำให้สงสัยว่า Ct brain เคสนี้ผู้ป่วยน่าจะมี brain mass ซ่อนอยู่

- เมื่อ CT brain with contrast ได้พบลักษณะ enhanced serpiginous vessels ตามพื้นที่ที่สงสัยก่อนนี้

- CTA brain ได้ diagnosis:  Cerebral arteriovenous malformation
