A 45-year-old woman with recently active SLE presented with fever with umbilicated skin papules and ulcers for 2 weeks. The lesions primarily occurred at face then had spread to trunk and extremities.

• She had received induction therapy for treatment of active proliferative LN within 2 months; including methylprednisolone 1 g IV x 3 days then prednisolone 60 mg/day for 4 week then 50 mg/day and cyclophosphamide 1 g IV q 4 week x 2 cycles

• Multiple encapsulated yeast with some budding yeast cell

Q3. What is the diagnosis?  Disseminated cryptococcosis 

• Cutaneous lesions are observed in approximately 5-10% of patients with disseminated cryptococcosis
• Skin lesions can persist for weeks or months before presentation, and are mostly found on the head, face, and neck (78%), but may be widespread
• The most common presentation is of translucent, dome-shaped, and umbilicated Molluscum contagiosum-like lesions (54%)
• Other presentations may include acneiform lesions, purpura, vesicles, nodules, abscesses, ulcers, granulomas, pustules, draining sinuses and cellulitis
• Definitive diagnosis can be made through skin biopsy
