A 46-year-old Thai woman presented with acute viral illness with anemic symptoms for 2 days. She had received blood transfusion once every year since the age of 10. The last time she received blood transfusion was 1 year ago. Plain films of skull (AP, lat) and CXR are as shown above.

1. Please describe positive findings of the plain films 

• Skull AP, lat:  Hair-on-end appearance with widening of the diploic space, thinning of the inner and outer table
• CXR PA upright:  Good exposure, no rotation, seen bilateral mass-like opacity at paravertebral area, rib expansion, and increased CT ratio 

2. What is the provisional diagnosis? 

• Major thalassemia with extramedullary hematopoiesis
