A 70-year-old man presented with right upper arm swelling after slipped in the bathroom. He complained being fatigue and loss of appetite for 3 months prior to this visit. The orthopedic surgeon on duty consulted you due to abnormal pre-operative laboratory results. 

Hb 8 g/dL, MCV 90 fL, WBC 5,800/uL, platelet 254,000/uL
BUN 30 mg/dL, Cr 2.5 mg/dL

ANSWERs

1. Findings from plain radiograph:

• Long bone:  diffuse skeletal osteopenia and osteolytic lesions with pathologic fracture of right proximal humerus 
• Skull:  numerous punched-out, osteolytic bone lesions at skull 

2. Provisional diagnosis:  Multiple myeloma (plasma cell myeloma)

3. Investigations for diagnosis:

• Bone marrow study Link to BM slide
• Identify clonal plasma cell: serum protein electrophoresis (SPEP), immunofixation/quantitative immunoglobulin, serum free light chain
• Bone survey:  10 locations (CXR, KUB AP, Pelvis AP, T-spine lat, LS spine lat, both humerus AP, both femur AP, both legs AP, skull lat)
  • Approximately, MM may involve spine around 60%, skull 40%, pelvis 30% and extremities 30%.
  • Possible findings at places other than skull and long bones 
    • Spine:  compression fractures 
    • Pelvis:  solitary plasmacytoma (large, expansile mass lesion)
