A 92-year-old man in bed ridden stage had developed fever with purple-colored urine in the urine bag for 2 days. Urinary tract infection was suspected.

1. Pathogenesis mechanism of the purple color of the urine

- "Purple urine bag syndrome (PUBS)" is rare and was first reported in 1978

- Risk factors:  women, chronically debilitated patients, long term indwelling urinary catheter, urinary tract infection

- Discoloration of the urine bag is due to the presence of indigo and indirubin pigments which precipitate and react with the synthetic materials of the catheter and urinary bag

2. The most likely causative organism(s)

- Providencia stuartii, Providencia rettgeri, Klebsiella pneumoniae, Proteus species, Escherichia coli, Enterococcus species, Morganella morganii, Pseudomonas aeruginosa.
